Update call implementation and custom ringtones

Description

TODO: add the permissions for calls into the permissions section

Pulls some things from upstream and attempts to match spec notification/call end/join behaviors a little better.
Adds an option for custom ringtones and ringbacks.
Adds some tests for things.
