Merge four keyword-overlapping learning clusters into denser consolidated entries; originals archived verbatim as a paper trail. - OpenCode plugin integration gotchas: 7 -> 1 - site/ tracked build output: 2 -> 1 - Contributor PR handling: 2 -> 1 - Drift/detection false positives: 3 -> 1 DECISIONS.md left atomic (each carries distinct rationale).
