Use std::map.emplace(_hint) where appropriate

As of stdlibc++-4.7.2 there is no support for the emplace or emplace_hint member functions for associative containers. According to bug 44436 this has been resolved and slated for release in libstdc++-4.8. Changes should be made upon release.
